WE NOW HAVE MORE SUPPLIES OF THE CHILDREN'S NASAL FLU, PLEASE CALL THE SURGERY TO BOOK AN APPOINTMENT FOR YOUR CHILD

PLEASE SEE THE INFORMATION IN THE 'FLU VACCINATIONS' TAB BELOW ON THE RIGHT FOR MORE DETAILS .

Don't forget, when you receive your flu vaccine at the surgery, we will also check if you have any other vaccines and checks due.

There is a nationwide shortage of Hepatitis A vaccine- we are trying to access supplies as best we can. Please bear with us and in the meantime if you are travelling please complete the form on this site in the 'Clinics & Services' tab under 'Travel Health' and we will contact you to discuss your individual needs.

 

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

New Identity Verification rules require forms of ID to be provided:

1. Photo ID eg Passport, Drivers license, Proof of age card, Bus pass

2. Address ID eg utility bill, Mobile phone bill, Buildings insurance, Contents insurance 

3. Personal ID ie Marriage or Adoption certificate if registration name is different to Photo ID

You will need to complete the forms on the following page 1. a registration form (GMS1) 2. a health questionnaire 3. a medication history and 4. a summary care form, which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
